What is cosmetic eyelid surgery?

Ear reshaping (Otoplasty)

Otoplasty is a procedure used to reduce large or protruding ears by setting the ears back closer to the head as well as molding, shaping and/or removing cartilage. The operation can be conducted on patients once the ears have reached, or nearly reached, their full size which typically is around 5-6 years of age. 

Ear surgery can dramatically change a person’s appearance simply by making protruding ears look more normal. Often the problem is caused by an undeveloped middle fold of the ear. There may be other deformities as well, making it necessary to perform several procedures on the ear at the same time. Otoplasty can reshape the ears, reduce their size, make them more symmetrical, and/or position them closer to the head.

Rhinoplasty

Nose surgery involves shaping the nose and is one of the most technically challenging cosmetic surgery operations: this operation should only be performed by experienced Consultant Plastic Surgeons.

Liposuction

Liposuction, is a technique to remove unwanted fat deposits. The unsightly distribution of body fat is usually due to an inborn tendency to deposit fat in one particular area of the body, most commonly around the hips. Other areas for fat deposits include; the neck, arms, tummy, loins, thighs, inner side of the knees and the ankles. 

Fat cells are believed not to be regenerated in adult life: their removal by liposuction will give a permanent change in contour and be independent of any changes in your body weight. 

It is important to understand that liposuction is not a treatment for obesity. The amount of fat that can be removed from a localised area of the body is limited by what is considered safe (maximum of 3 litres) and a natural limitation when no further fat can be removed. Therefore it may not be possible to slim down an area as much as you might wish. Further treatments may be carried out in the same area after six months. Where the skin is inelastic and loose, Liposuction will tend to leave the skin more lax and it may be recommended that a skin excision be carried out to correct this, either at the same time as the liposuction, or as a second procedure. This is most likely in the abdomen, after pregnancy or weight loss, the buttocks or the neck. Please also note that dimples and wrinkles of the skin, sometimes called cellulite will not be improved by liposuction.

Post weight loss skin tightening / Body tucks

After losing a great deal of weight in the thigh area, many patients continue to experience problems related to excess skin remaining on the body. These difficulties r can generally only be solved through excess skin removal. Thigh lift is a common component of excess skin removal following bariatric surgery.

Nymphoplasty (Vaginal Reduction & tightening)

Nymphoplasty is a new type of intimate surgical procedure which involves the reduction of the inner labia, most often for aesthetic purposes. Though more and more frequent, this operation remains a subject for debate. Doctissimo takes a closer look.

Half-hearted requests

Hypertrophy of the inner labia can be congenital (from birth) but more often, this over-development in the body occurs during puberty and is made worse by giving birth. On the other hand, after menopause, inner labia tend to become atrophied and are obscured by the outer labia.

Note on Anatomy

The vulva displays parts which are visible like the mons veneris and the outer labia but also has hidden parts like the inner labia (or nymphae), the vulval vestibule where the urethral opening is situated, the vagina and the clitoris. In contrast with the outer labia, the inner labia are not covered with hair but they do contain many sebaceous follicles and very sensitive nerve endings. They connect together at the top and in front to form the clitoral hood and frenulum clitoridis, but do not necessarily join at the base. Varying in size, they measure at least 3cm long. Their external edges surface and protrude beyond the vulvar slit. 

But the requests for the procedure often come from young girls, who have not had children and who are “affected not only aesthetically but physically, through difficulties involving pain during sex, when practicing certain sports (cycling, horse-riding) or because of wearing too tight clothes”, explains Dr. Laurent Benabida, a plastic surgeon.1 
In fact, exchanges in online forums show that certain changes in clothing (trend for tight jeans, thongs...) and the influence of porn films in which stars display juvenile pubic areas have caused a rise in requests for nymphoplasty.

V-shaped nymphae

The goal here is to reduce the inner labia by taking away the excess submucous tissue. Lasting at least 45 minutes, this operation can be carried out under local anaesthesia, but this is a very sensitive area of the body and the woman is therefore obliged to stay in the gynaecological position. This is the widely preferred option in the United States, but in France, patients opt for general anaesthetic. The procedure is completed in the day and does not require a night in hospital.2. 
The initial technique involved removing a crescent shape from the surface of the skin. In this case, total amputation of the inner labia (nymphectomy) is possible. But the large resulting scar comes into direct contact with underwear (and there is therefore a higher risk of infection, hematoma and painful post-operative complications...) so a different technique is general preferred instead; central or upper resection in a “V”3 or even two resections in a “V” of the external parts (a VV technique). This means the scar is in the lesser exposed areas and therefore allows sexual relations to begin again sooner. 

The post-operative effects generally require straightforward painkillers (paracetemol based) to sooth the stinging sensations, daily localised treatments to apply until the resorbable stitches fall out, and also means sex must take a back seat for four to six weeks. During this period, tampon use is forbidden. 

Be careful, however, as the post-operative complications are not insignificant, even if they are very rare.2 These include: stitches coming undone due to injury, slow healing, localised infection, hematoma and even pain during penetrative sex or changes in sensitivity in the intimate area. The two latter are thankfully mostly transitory. When confronted with these risks, it is preferable to talk to a doctor who is familiar with this kind of operation.

A controversial surgical procedure

Certain doctors and sex therapists are worried about the increasing demand for these operations. According to the experts, the dictated aesthetic norms (mainly pedalled by porn films) lead to complexes which shouldn’t be there. Some sex therapists think that the reduction of these regions of the body where many sensory nerves are located is risky in terms of feeling, and they recommend that the technique should only be used for women for whom the size of their inner labia causes physical problems, affecting walking and therefore having an impact on everyday life.

Day of Procedure

Hair transplantation is an outpatient procedure that typically lasts 3-8 hours. (In rare cases the procedure takes a little longer depending on the number of follicular units being transplanted.) In general, you will feel very minimal discomfort. Our doctors use mild sedatives and a local anesthetic. During the procedure, you simply need to relax and pass the time. 

Before you begin, your doctor will review your crown density and confirm details the two of you have discussed prior to any kind of surgery for hair loss, including the specific hairline you want and expectations of the results. Next, your doctor will review the procedural steps detailed below and answer any questions you may have.

Step 1: Donor Area Preparation

A donor strip is taken from the back or sides of the head, where hair is genetically resistant to hair loss. The hair surrounding the donor strip is taped or secured while the hair in the donor area is trimmed. The donor area is then numbed with a local anesthetic.

Step 2: Harvesting

The strip of donor tissue is surgically removed and the skin in the donor area is sutured together. Once healed, it will typically leave a very fine cosmetic line or scar which will be covered by your existing hair. Even immediately following the suturing of the donor area, the affected area is hardly visible.

Step 3: Separating

A team of highly-trained medical assistants, working with stereo microscopes, divide the hair from the donor strip into natural follicular groupings. (Your hair naturally grows in groupings of one, two, three and four hairs.)

Step 4: Create Recipient Sites

The creation of recipient sites is the most artistic part of the procedure. Your doctor will make incisions to the depth of each follicle that follow the exact direction and angle of your hair growth. There is no substitute for experience when it comes to creating the recipient sites.

Step 5: Placing

Follicular units are carefully placed into the recipient sites by the physician's experienced medical team. The one- and two-hair follicular units are primarily used in the frontal area of the hairline while the three- and four-hair follicular units are usually placed behind them and on top of the head for more density.

What to expect
